The Hospital at Home RN is responsible for providing high-quality
patient care to various patient populations within the framework of
the Medically Home model. This position requires population
specific competencies. Adheres to National Patient Safety Goals as
appropriate based on the level of patient contact this position

* Current licensure as a Registered Nurse (RN) in the State of
Oklahoma or current multistate license from a Nurse Licensure
Compact (eNLC) member state. * 2 years of Registered Nurse
experience in an acute care setting. * ADN required; BSN preferred
* Current BLS certification upon hire, or completion within 30 days
of hire required. * Must be able to communicate effectively in
English. * Demonstration of core competencies will be completed
within 90 days. * Completion of unit specific competencies within
12 months.
The Hospital at Home RN responsibilities includes, but are not
limited to: * Utilizing monitoring, assessment, planning,
intervention and critical thinking skills to care for acute care
patient populations in the home * Navigating patients and families
across the continuum of the Medically Home phases of care *
Monitoring and responding to biometric data per Medically Home and
INTEGRIS Health protocols * Conducting home and audio/visual
patient care visits as directed by the care team and/or protocol *
Ensuring all interventions in the patient's care plan are
implemented efficiently with a patient centered approach *
Leveraging the resources and expertise of the Medically Home team
and supplier partners to implement collaborative workflows,
standards, policies, protocols, guidelines and documentation
systems to support safe, reliable, high-quality, evidence-based
care with clinical protocols as the foundation * Adhering to the
clinical protocols and be willing to obtain the necessary training
to provide care within the context of the providing evidence-based
care * Promoting professional practice and a culture of safety;
willingness to engage in process improvement efforts Clinical Roles
and Responsibilities: * Providing protocol driven assessment, care
coordination and interventions as appropriate informed by Medically
Home and INTEGRIS Health protocols for diverse patient populations.
* Exhibiting sound clinical decision making and critical thinking
skills * Maintain and practice in accordance with nursing
guidelines per state and hospital regulations * Facilitating
admissions to Medically Home utilizing MH guidelines * Serving as a
role model for professional nursing practice * Utilizing nursing
knowledge to identify, prevent and/or solve complex acute and/or
recurring patient care problems * Sharing expertise with the
multidisciplinary team and serve as a resource to staff in area(s)
of expertise * Supporting research efforts to advance knowledge and
promote evidence-based practice * Preforming other duties and
responsibilities as assigned by the Nurse Manager Quality and
Safety Responsibilities: * Ensure compliance with reporting
requirements for Sentinel Events and Serious Incidents *
Participate in quality and safety investigations and root cause
analyses * Take appropriate action to ensure patient safety as a
result of any investigation; and * Serve as a resource in quality
improvement processes and technique The Hospital at Home RN reports
to the appropriate manager, director, or vice president. This
position may have additional or varied physical demand and/or
